Hawaii Medical Use of Marijuana
Assistance in the process of obtaining a Hawaii Medical Use of Marijuana card for residents of Hawaii who have a qualifying condition.
The state of Hawaii recognizes the following conditions:
Severe Pain
Severe Nausea
Persistent Muscle Spasms
Seizures & Epilepsy
Cancer
Glaucoma
HIV+ / AIDS
Crohn’s Disease
Multiple Sclerosis
Cachexia (extreme weight loss)
PTSD
Generally a patient will have to have suffered from one (or more) of the above for at least 6 months in order to qualify for the 329 Card.
How it works
Book an evaluation session. Bring in any related medical records & Rx pill bottles, if you have them. You will be evaluated and undergo orthopedic and neurological examinations at the office. This creates a new medical record. If your condition is found to be a qualifying one, you will fill out a packet for the Hawaii DOH, which is then sent to the state for processing and 329 Card issuance.
What it costs
Initial evaluation and examination $120
Processing fee $60
State of Hawaii fee $38.50 (the state fee must be paid online or with a credit card)
